Background

  • Immature red blood cells; reflects bone marrow erythropoietic activity

Normal Values

  • Normal: 0.5-2.5% (absolute count 25,000-125,000/mcL)
  • Reticulocyte production index (RPI) corrects for degree of anemia

Interpretation

  • Elevated reticulocytes (>2.5%): appropriate marrow response to blood loss or hemolysis
  • Low reticulocytes in the setting of anemia: marrow failure (aplastic anemia, B12/folate deficiency, infiltration, medications)
  • Key test in the evaluation of anemia: helps distinguish decreased production from increased destruction/loss
