Reticulocyte count
Background
- Immature red blood cells; reflects bone marrow erythropoietic activity
Normal Values
- Normal: 0.5-2.5% (absolute count 25,000-125,000/mcL)
- Reticulocyte production index (RPI) corrects for degree of anemia
Interpretation
- Elevated reticulocytes (>2.5%): appropriate marrow response to blood loss or hemolysis
- Low reticulocytes in the setting of anemia: marrow failure (aplastic anemia, B12/folate deficiency, infiltration, medications)
- Key test in the evaluation of anemia: helps distinguish decreased production from increased destruction/loss
